In the role of Patient Navigator, you will manage the daily operations and workflows of the York Oncology program in conjunction with physicians and administrative leadership.
Some essential functions of this role include:
- Responsible for acting as a patient liaison for the program
- Works closely with Oncology physicians to ensure support of all new patients
- Works with referring physician offices and clinics to manage patient needs
- Assist new and existing patients in understanding their diagnosis, treatment options and the resources available
- Develop and distribute educational materials for patient use
- Plans and develops patient-centered programs. Reinforces patient education and directs patients and families to available resource and supportive services
- Performs program quality assessment checks, audits and reviews along with statistical reporting
Requirements for this position include a Bachelor of Science in Nursing with a minimum of 3 years of clinical experience, preferably in Oncology and/or Urology. OCN Certification or ability to attain within 24 months of employment required. Previous patient navigation experience desired.
